摘要
目的探讨应用计算机辅助细胞检测系统(CCT)、人乳头瘤病毒抗酸定性检测(HPV-DNA)配合阴道镜检查对子宫颈癌前病变的诊断价值.方法收集本院2003年4月～2004年10月妇科门诊患者子宫颈涂片1025份采用CCT检测做出诊断,并检查HPV-DNA,对阳性病例83例再阴道镜下取活检.结果异常涂片76例,占7.4%,其中ASCUS29例,LSIL28例,HSIL13例,宫颈鳞癌5例,腺癌1例.CCT与病理检查符合率为80.7%,敏感性95.5%,特异性98.6%,HPV16/18DNA检出率随着病变程度的加重呈上升趋势.结论采用CCT及HPV-DNA初筛,异常者配合阴道镜下活检,能提高子宫颈癌前病变检出率.
Objective To investigate the efficiency of Computer - assisted cytology test （CCT） and humin papilloma viruse - DNA （HPV - DNA） cooperate with vaginoscopy in diagnosis of cervical intraepithelial neoplasia （CIN）. Methods CCT and HPV - DNA were used to examine the endocerical Scraping Smear in 1025 cases of patients with gynepathy in out - patient department form Apr. 2003 to Oct, 2004, and cooperated with biopsy and vaginoscopy in 83 positive cases, Results Abnormal smears were 76 cases （7.4%）, 29 cases were ASCUS, 28 cases were LSIL, 13 cases were HSIL, 5 cases were sguamous cell carcinoma of cervix and one was adenocarcinoma. CCT accorded with pathologic examination rate was 80.7%, The sensitiveness was 95.5%; the specifieness was 98.6%. The positive rate of HPV - DNA was elevated with disease. Conclusion CCT and HPV - DNA are sifting to CIN, and positive cases cooperated with biopsy under vagninoscopy, can predict the CIN.
