摘要
目的 报告Triamcinolone玻璃体腔内注入的可视化玻璃体手术对白内障合并视网膜脱离的临床疗效。方法 将2000年1月～2002年1月和2002年3月～2003年3月的白内障合并有视网膜脱离的32例(32只眼)、21例(21只眼)分别采用常规的白内障超声乳化联合玻璃体切除和Triamcinolone玻璃体可视化手术作为A组和B组，对比其术后并发症。结果 B组前房内纤维素渗出及虹膜后粘连及晶状体后囊混浊发生率明显低于A组，两组对比有显著性差异(P＜0.01)。而视网膜解剖复位和视网膜脱离复发率、术后高眼压、青光眼、前房出血二者并无明显差异(P＞0.05)。结论 Triamcinolone对白内障联合玻璃体切除术后并发症，特别是葡萄膜炎性反应有强烈抑制作用。
Objective To report the clinical outcome and complications of intravitrous injections of triamcinolone in patients undergoing pars plana vitrectomy on the visibility of hyaloid surgery and pbeceemulsification for treatment of cataract combined retinal detachment. Methods All casees of cataret combined retinal detachment were divided into the A＆B groups. The A group included 32 patients （ 32 eye ） were treated during 2000 Jan to 2002 Jan undergoning traditional phaeeemulsification combinded pars plana vitrectomy. The B group included 21 patients （21 eye ） during 2002 Mar to 2003 Mar undergoing pars plana vitrectomy （PPV） on the visibility of hyaloid surgery, The A group was compared with the B group in postopertive complication. Results The complicantion of anterior chamber fibrin exudation and posterior synechia of iris and posterior capsule opacificatio in the B group is lower than in the A group , it varies significantly between the groups（ P 〈0. 01 ）. Incidence of retinal redetachment, anatomice reattachment rate, hyphema, transient postoperative intraocular pressure increase and glaucoma did not vary significantly between the groups （ P 〉 0. 05）. Conclusion The intravitreal injection of triamcinolone can decrease the postoperative complication of vitrectomy combinded phaceemulsification and it can decrease uveitis violently.
