摘要
通过７４例胃溃疡急性穿孔手术治疗临床资料分析，主张对该类病人积极施行急诊胃大部切除术。其适应症是：（１）术前无难以纠正的休克；（２）重要脏器无严重功能障碍；（３）腹腔无局限性脓肿。消化道重建方式以Ｂ－Ⅰ式为首选，必须行胃空肠吻合时，推荐Ｒｏｕｘ－Ｙ和Ｂ－Ⅱ式加Ｂｒａｕｎ吻合术。穿孔修补和溃疡局部切除术仅用于不宜行胃大部切除者。
In analysis of surgical management of 74 patients with acute perforation of gastric ulcer,the per-formance of subtotal gastrectomy should be considered tvith indications of(1)no incoercible shock beforeoperation;(2)no dysfunction of the inain organ;(3)no circumscribed abscess in the peritoneal cavity,Bill-roth I is the method of choice in reconstruction of digestive tract.But if gastrojejunostomy must be under-taken,Billroth Ⅱ and Braun jejunojejunostomy are recommended.Repairing of perforation or resection ofulcer is only used when subtotal gastrectomy cannot be performed.
