摘要
目的探讨采用珊瑚多孔羟基磷灰石（ｃｏｒａｌｐｏｒｏｕｓｈｙｄｒｏｘｙａｐａｔｉｔｅ，ＣＨＡ）材料行眶缘塌陷充填再造术的效果及安全性。方法用ＣＨＡ行眶缘塌陷充填再造术７例。ＣＨＡ规格为２００μｍ孔径的内联多孔板块状材料，尺寸为１．０ｃｍ×１．５ｃｍ×５．０ｃｍ、０．６ｃｍ×１．５ｃｍ×５．０ｃｍ、０．４ｃｍ×１．５ｃｍ×５．０ｃｍ三种。术前参照健侧眶制作橡皮泥模型后，再以ＣＨＡ材料塑形拼接成所需形状。手术切口设计为面部上１／３除皱切口及下睑袋延长切口处，暴露塌陷区后充填已塑形的ＣＨＡ材料。结果用ＣＨＡ行眶缘塌陷充填再造术７例，均获满意效果。随访３～１２个月，平均６．６个月，未见植入物脱出、移位等并发症。结论ＣＨＡ是一种良好的骨替代材料，明显优于自体组织及其它人工材料。
ObjectiveTosearchfortheeffectandsafetyofcoralporoushydroxyapatite(CHA)implantedmaterialinsrbitalrimreconstruction.Methods7casesoforbitalrimreconstructionwere-treatedwithCHAwhichisaplateformmasswithaninterconnectedsystemofcanalsofapproxima-tely200μminsixe.Ithasthreetypes:1.0cm×1.5cm×5.0cm,0.6cm×1.5cm×5.0cmand0.4cm×1.5cm×5.0cm.Accordingtotheshapeofacontralateralhealthyorbit,arubberputymodelwasmade-beforetheoperation.Anincisionwasdesingnedattheupper1/3ofthefacefortheelimnationofwrinklesoraprolongedincisionoflowerlidbagplasticsurgerywasmade.Aftertheexposureofthedefectivepartoftheorbit,acorrectlymoldedCHAtamponadewasinserted.ResultsThjeorbitalrimreconstructionin7caseswithorbitaldefectwassuccesfulytreatedwithCHA.Duringafolow-upof3～12monthe(average6.6monthes),therewasnoextrusionormigrationofimplant.ConclusionsCHAisagoodmaterialforsubstitutionofbonegraftandismuchbeterthanautogenoustissueorotherartificialmaterials.
