摘要
目的:研究肝纤维化的发生机制和诊断方法,为治疗提供理论依据。方法:分离动物肝间质和实质细胞,观察细胞因子、细胞外间质(Extracellularmatrix,ECM)、中药等对细胞及动物肝脏中胶原mRNA表达的影响;提纯ECM成分,制备抗体并建立检测方法,以血清中ECM含量变化判断早期肝纤维化的形成。结果:肝纤维化发生时,肝内细胞胶原mRNA表达增强,细胞因子有重要调节作用。血清中ECM水平与肝组织纤维增生变化相平行。结论:肝纤维化大量ECM增生原因为其基因激活转录所致,这种激活受细胞因子调节。ECM的血清学检测可作为早期诊断和监测临床治疗情况的指标。
Objective:To study the mechanism and early diagnosis of liver fibrosis and provide a theoretical basis for its therapy.Methods:Hepatic Ito cells,hepatocytes,endothelial cells and Kupffer cells were cultured.The effects of cytokines,extracellular matrix(ECM) and some traditional Chinese medicines on cell collagen mRNA expression were studied.Some collagens,glycoprotein and proteoglycans were isolated and purified,and corresponding assaying methods were set up. Results:The expression of collagen mRNA was increased in vivo and in vitro studies on cultured cells and in rat fibrotic liver.Serum levels of ECM were parallelled with the degrees of hepatic injuries and fibrogenesis.Conclusions:The increased deposition of ECM proteins is due to the increased transcription of ECM genes,which are regulated by cytokines,such as TGF-β,PDGF, EGF,TNF-α,IL-1,IL-6,IFN-γ,etc.Assessment of serum ECM contents may be used as an index for early diagnosis and treatment of liver fibrosis.
