摘要
目的研究急性淋巴细胞白血病免疫分型中抗原的异常表达。方法用多参数流式细胞仪对76例初诊的急性淋巴细胞性白血病(ALL)患者进行免疫分型。结果76例ALL中B系ALL70例占92.1%,T-ALL占6例。70例B系ALL中CD45-或CD45±表达占75.5%,显示该白血病细胞具有异质性;CD38弱表达占60%,CD10强表达占40%;B系ALL表达髓系抗原以CD33最高达25.7%,CD13次之达24.2%,CD15和CD11b较少见,分别占5.7%和1.4%。结论急性淋巴细胞白血病流式细胞术免疫分型中抗原异常表达,不但在初诊时有助于白血病克隆的识别、门的设定、白血病细胞数的定量,而且能在形态学缓解后作MRD检测,为白血病复发监测提供重要手段。
Objective To study the significance of antigen abnormal expression with immunophenotyping in acute lymphoblastic leukemia(ALL). Methods Immunophenotyping was detected in 76 prime patients with acute leukemia by plufi-parameter flow cytometry (FCM). Results There were 70 patients with B-ALL and 6 patients with T-ALL in 76 ALL patients. Among 70 B-ALL patients, 75.5% were CD45- or CD45 ~expression, and showing heterogenous B-ALL cells; 60% of patients were CD38 low expression; 40% of patients were CD10 high expression. For myeloid antigens, CD33 was the hightest to 25.7%, and CD13 was up to 24.2%, but CD15 and CD 11 b were uncommon, the positive rates were 5.7% and 1.4% respectively. Conclusions The antigen abnormal expression in ALL with multi-parameter immunophenotyping can be not only useful for leukemic clone, gating and quantitative leukemic cells, also able to detect MRD after morphology remission, so it offers an important method for detecting leukemia recurrence.
