组织工程活性皮肤在供皮区创面的应用效果 被引量:2

Clinical application of tissue engineeried skin for the wound on donor site

摘要 目的:组织工程活性皮肤应用于供皮区创面的有效性及安全性检测。方法:选取西安铁路中心医院烧伤整形科2003/2004需植皮患者16例,供皮区部分创面采用组织工程全层皮肤覆盖,选取邻近创面作为对照,对照区采用凡士林油纱布覆盖。治疗期间观察创面有无炎症反应、有无渗出及疼痛的轻重情况;随访1个月,对患者创面进行有效性及安全性评估:观察创面的愈合情况及不良反应,以能否揭出纱布为愈合指标,计算应用部位与对照的愈合时间。结果:按实际处理分析,纳入患者20例,进入结果分析16例。①创面愈合情况:患者术后病情平稳,创面无明显炎性反应及渗出,可见组织工程皮肤在创面存活,创面愈合时间与对照相比明显缩短2~4d。愈后未发现任何副作用。②平均愈合时间:应用区域短于对照区域[(7.51±1.68),(9.24±2.84)d,P<0.05]。结论:应用组织工程皮肤可促进供皮区的愈合,更快的治愈供皮区创面,明显缩短平均愈合时间,降低患者的疼痛、水疱形成等不良反应的发生率;为组织工程皮肤的临床应用提供了切实可行的方法。 AIM: Detection of effectiveness and reliability of the tissue engineered skin (TE skin) in the application on wound donor site. METHODS: Sixteen patients with donor wounds, who were treated at the Department of Burn and Plastic Surgery, Xi'an Railroad Central Hospital from 2003 to 2004, were selected and treated with TE skin on part of the wound donor site, and the other near wound were selected as control group. The petrolatum gauze covered the control region. The inflammatory reaction of the wound, bleeding and ache was observed during the treatment period. The effectiveness and the safety of TE skin had been assessed in patients during 1-month follow-up. The healing condition and the bad reaction of the wound were observed, and the healing standard was that the carbasus could be torn off or not. The healing time hetween application part and control was calculated. RESULTS: According to the actual dispose analysis, 20 patients were included, and 16 cases were involved in the resuh analysis. ① Healing condition of wound: The patient's condition 'after operation was stable without significant inflammatory reaction and bleeding on the wound. The existence of wound was seen on the TE skin. The healing time of wound was significantly shortened to 2-4 days compared with that of the control. There was not any ill effect after healing. ② Average healing time: It was shorter in the application region than that in the control region [(7.51±1.68), (9.24±2.84) days, P 〈 0.05]. CONCLUSION: TE skin can accelerate the healing on the donor site, heal the wound on the donor site more rapidly, shorten the average healing time significantly, and reduce the incidence of bad reaction, ache and the formation of blister in patients etc. The practicable method is provided for the clinical application of TE skin.
