摘要
目的观察参附注射液(SF)对体外循环期间胃黏膜pH值(pHi)、内毒素血症及全身炎性反应的影响。方法选择临床心内直视手术患者60例,随机分为对照组和SF组,SF组于切皮时静注SF0.5mL/kg,再取1.0mL/kg从Grasby泵注入,对照组则输入等量生理盐水。分别于于切皮前(S0)、主动脉开放1h(R1)、2h(R2)后测定胃黏膜pH值(pHi)、血浆内毒素(LPS)、及白介素(IL-6)水平。结果两组R1和R2时,pHi值显著低于S(0均P<0.01)、LPS和IL-6显著高于S(0均P<0.01);SF组R1和R2时pHi值显著高于对照组(均P<0.01),LPS和IL-6显著低于对照组(均P<0.01)。结论SF通过改善体外循环期间胃肠灌注及氧合,减轻内毒素血症及全身炎性反应。
[Objective] To investigate the effect of shen-fu on gastrointestinal intramucosal pH(pHi), endotoxemia and systemic inflammatory during cardio-pulmonary bypass. [Methods] 60 patients undergoing elective heart surgery were divided into control group and SF group randomly. Before CPB, The SF group was treated with SF 0.5 mL/kg by intravenous injection, then continued with SF1.0 mL/kg injected from Grasby pump, The control group was injected with normal saline in equal volume. Intramucosal pH value (pHi), blood LPS and IL-6 were measured before CPB, 1 h after artery was opened, 2 h after artery was opened. [Results] In both groups pHi at R1, R2 was lower than that at So significantly (both P 〈0.01). Blood LPS, blood IL-6 at R1. R2 were higher than those at So significantly (both P 〈0.01 ). In SF group, pHi at R1. R2 was higer than that in control group significantly (both P 〈0.01 ), Blood LPS, blood IL-6 at R1. R2 were lower than those in control group significantly (both P〈0.01). [Conclusion] SF can improve gastrointestinal perfusion and oxygenation, then reduce endotoxemia and systemic inflammatory during cardio-pulmonary bypass.
