摘要
目的研究腹腔灌注联合全身静脉化疗预防中晚期结肠癌术后腹腔局部复发、肝转移的疗效及其对生存率的影响。方法62例DukeB～C2结肠癌患者根治术后进行腹腔灌注联合全身静脉化疗,57例DukeB～C2结肠癌患者根治术后进行全身静脉化疗,比较两组患者腹腔局部复发、肝转移率及生存率。结果联合化疗组62例和对照组57例均顺利完成6周期化疗,联合化疗组的肝转移率(8/62,12.90%)和局部复发率(7/62,11.29%)均明显低于对照组(分别为18/57,31.58%;23/57,40.35%),差异具有显著性(分别为P<0.05;P<0.01)。联合化疗组1年生存率为93.55%、2年生存率为80.65%、3年生存率为69.35%;对照组1年生存率为89.47%、2年生存率为63.16%、3年生存率为50.88%,联合化疗组2、3年生存率明显高于对照组,差异具有显著性(分别为P<0.05;P<0.05)。结论腹腔灌注联合全身静脉化疗能有效预防中晚期结肠癌术后局部复发和肝转移,显著提高总体生存率。
[Objective] To evaluate the effect of intraperitoneal chemotherapy combined with system chemotherapy for advanced stage colon cancer following radical operation on local recurrence, hepatic metastasis and overall survival rate. [Methods] 62 patients with Duke B～C2 colon carcinoma undergoing intraperitoneal chemotherapy combined with system chemotherapy after radical operation, 57 patients with Duke B～C2 colon carcinoma undergoing system chemotherapy after radical operation, local recurrence, hepatic metastasis and overall survival rate between them were analysed. [Results] Both group underwent 6 cycles chemotherapy, the local recurrence and hepatic metastasis rate of combined chemotherapy group are lower than that of control group （7/62, 11.29% vs 23/57, 40.35%; 8/62,12.90% vs 18/57, 31.58% respectively）, and had significance （P 〈0.05; P 〈0.01 respectively）. The 2- year, 3-year survival rate of combined chemotherapy group are higher than that of control group （50/62, 80.65% vs 36/57, 63.16%; 43/62, 69.35% vs 29/57, 50.88% respectively）, and had significance （P 〈0.05; P 〈0.05 respectively）. [Conclusion] The intraperitoneal chemotherapy combined with system chemotherapy could effectively prevent local recurrence and hepatic metastasis for patients with advanced stage colon cancer following radical operation, and improve survival rate.
