摘要
目的探讨大鼠急性胰腺炎(AP)肺损伤时,肺组织损伤程度与组织巨噬细胞炎症蛋白-2(MIP-2)、肿瘤坏死因子-α(TNF-α)基因表达的关系。方法胰胆管逆行注射5%脱氧胆酸钠(DCA)复制大鼠AP模型,观察胰腺和肺组织病理学改变,使用RT-PCR法检测肺组织中MIP-2 mRNA、TNF-αmRNA的表达。结果AP 3 h、AP 6 h组胰腺和肺组织镜下病理评分明显高于相应对照组。AP 6 h组肺组织MIP-2 mRNA表达明显上调。胰腺病理评分与肺组织病理学评分、肺组织MIP-2基因表达呈正相关;肺组织MIP-2与TNF-α的基因表达之间也呈明显正相关。结论肺组织MIP-2基因表达上调,以及与TNF-α的相互诱生,可能是导致急性胰腺炎中性粒细胞在肺组织浸润和活化,进而导致肺损伤的重要机制之一。
Objective To investigate the relationship between the gene expression of macrophage inflammatory peptide - 2 (MIP - 2) and TNF- α in lung and the acute pancreatitis- associated lung injury in rats. Methods 18 SD rats were divided at random into four groups: AP 3 h group( n =6), AP 6 h group( n =6), control 3 h group( n = 3), control 6 h group( n = 3). Rat AP model was induced by intraductal administration of 5% sodium deoxycholate(DCA). The severity of pancreatitis and lung injury were measured by histopathologic scores. MIP- 2 mRNA and TNF- α mRNA in lung were measured by semi quantitative RT- PCR. Results The histopatholggic scores of pancreas and lung were higher in AP 3h group than in ,control 3h group( 14.33 =t 0.82 vs 2.00 =t 2.65, P 〈 0.01 ; 1.50 =t 0.84 vs 0.33 =t 0.58, P 〈 0.05) ; those were higher in AP 6 h group than in control 6 h group( 12.50 =t 4.28 vs 1.33 =t 0.58, P 〈 0.01 ;2.17 =t 0.75 vs 0.67 =t 0.58, P 〈 0.01 ). The lung MIP- 2 mRNA up- regulated in AP 6 h group(0.16 =t 0.10 vs control 6 h group 0.01 =t 0.01, P 〈 0.05). The lung histopathologic scores and MIP- 2 mRNA levels were positively correlated with the pancreas histopathologic scores( r = 0.47,0.59, P 〈 0.01 ). The lung MIP - 2 mRNA levels were also correlated positively with the lung TNF- α mRNA levels ( r = 0.59, P 〈 0.01). . Up- regulation and interaction of MIP- 2 mRNA and TNF- α mRNA in lung perhaps play a key role in the development of acute pancreatitis- associated lung injury by recruiting PMNs into lung and activating PMNs .
