摘要
目的探讨三维可视化及虚拟手术技术应用于肝脏外科的准确性及实际意义。方法利用灌注成像HCT扫描法对35例肝脏标本及其肝内主要管道系统进行扫描,采集二维图像数据;利用3DMED软件对二维图像数据进行三维可视化重建,并对重建肝脏模型的体积与肝脏标本实际情况进行对比研究;在准确重建的三维模型上使用虚拟手术工具对其进行模拟手术分割,并按照虚拟分割层面,对相应肝脏标本进行实际分割。结果重建后的肝脏模型三维效果逼真,立体感强,可任意角度旋转、观察;能够显示肝内各主要管道系统的空间位置关系,并较准确地反映肝脏体积及肝内管道系统的真实情况。虚拟手术系统对重建的肝脏模型可进行任意曲面切割,断面能显示所遇到之管道结构,并与实际肝脏标本的切割断面情况相符。结论三维可视化重建基础上的虚拟手术技术是肝脏外科有效的辅助工具;对制定合理的手术预案具有重要的临床价值。
Objective To explore the significance of three dimensional visualization and virtual surgery system in the planning of hepatic surgical project by 3D reconstruction and virtual division. Methods Thirty-five liver specimens were scanned to collect 2D DICOM data, and 3D model was reconstructed by 3D MED soft. The volume of the reconstructed hepatic models was compared with that of the liver specimen. Virtual cutting were done on reconstructed hepatic models, and liver specimens were cut according to the virtual cutting plane. Results The verisimilitude of the reconstructed 3D liver models were demonstrated, and the models could show the spatial relationship among main tracts and forecast hepatic volume exactly. Curve incision can be made in the 3D liver models at will by virtual surgery system, and the conduct frameworks at break surface be displayed, which matched with the hepatic specimen well. Conclusions The technique of virtual surgery based on 3D visualized recon struction is an available assistant tool for hepatic surgery, which is important in planning reasonable operation scheme.
