摘要
目的了解风湿性心脏病患者心肌中核转录因子表达的特点,探讨核转录因子与风湿性心脏病心肌纤维化的关系。方法风湿性心脏病患者30例,术中取左室乳头肌,以成年正常乳头肌10例为对照,苏木素-伊红(HE)、Masson染色检测心肌胶原,免疫组织化学方法观察核因子-kappa B(NF-κB)、活化蛋白-1(AP-1)在心肌中的表达,均用图像分析仪定量分析。结果风湿性心脏病心肌组织中间质胶原纤维明显增生。NF-κB与AP-1在风湿性心脏病心肌中阳性表达增强,表达强度与心肌纤维化面积呈正相关(NF-κB:r=0.8945,AP-1:r=0.9011,P<0.01),与NYHA心脏功能分级呈正相关(NF-κB:r=0.7247,AP-1:r=0.7879,P<0.01);在正常心肌中表达呈弱阳性或阴性。结论NF-κB与AP-1均在风湿性心脏病心肌纤维化形成中起作用。
Objective To investigate the effects of nuclear transcription factor-kappaB(NF-kB)and activated protein-1(AP-1)on rhcumatic myocardial fibrosis.Methods Thirty rheumatic myocardial specimens and 10 normal samples were examined by Masson's staining.immunobistochemical staining adn image analysis.Results Both NF-kB and AP-1 were expressed in the nucler of myocardium with rheumatic heart disease(RHD).The degree of positive staining in myocardiurn with RHD was significantly correlated with the cardial collagen volume(NF-kB:r=0.8945,AP-1:r=0.9011,P〈0.01)and the NYHA functional classes(NF-kB:r=0.7247,AP-1:r=0.,7879,P〈0.01).Conclusion Both NF-kB and AP-1 may play a key role in rheumatic myocarlial fibrosis.so a new thcrapeutic strategy to prevent or regress myocardial fibrosis at molecular level may be found.
