骶髂关节注射皮质激素治疗脊柱关节病 被引量：5

摘要 目的了解骶髂关节(SIJ)内注射皮质激素联合缓作用药治疗血清阴性脊柱关节病(SpA)的疗效。方法病人取俯卧位,CT导引将腰穿针穿刺进入SIJ注入醋酸泼尼松龙(50～100)mg和2%利多卡因1ml,记录注射前后的临床资料。结果22例中20例(90.9%)腰背痛减轻。夜间痛、晨僵、SIJ压痛、骨盆挤压试验阳性、“4”字征阳性例数较术前明显减少(P<0.01)。ESR及CRP升高的例数和程度减少或降低(P<0.05)。随诊1.6年,12例(54.5%)腰背痛程度较术前减轻50%,复查SIJ炎均未向上发展。结论SIJ内注射皮质激素不失为NSAIDs治疗无效或不能耐受的SpA病人缓解腰背痛的一种手段,联合应用缓作用药可控制病情活动。
