摘要
为了探讨大脑半球软化病灶和丘脑萎缩的关系，根据有无丘脑萎缩征象，将１３７例各种病因所致的大脑半球脑软化病灶患者分为萎缩组（７７例）和对照组（６０例），分析脑软化病灶的形态学特征和丘脑萎缩的关系。结果：累及顶叶、枕叶和基底节区的脑软化病灶可出现丘脑萎缩征象，且萎缩组脑软化病灶的体积大于对照组。出现丘脑萎缩的最早时间为起病后３个月，丘脑萎缩程度和病期呈负相关。丘脑萎缩常和脑干华勒变性同时出现，但枕叶病变者所致的丘脑萎缩大多数不伴有脑干华勒变性。结果显示：顶叶病变所致的丘脑萎缩可能与逆行性神经元细胞坏死有关；枕叶病变所致的丘脑萎缩可能和丘脑的缺血坏死有关，基底节区病变所致的丘脑萎缩则可能与丘脑的直接破坏和（或）逆行性神经元细胞坏死有关。
Therelationshipbetweencerebralencephalomalaciaandthalamicatrophywasstud-ied.MaterialandMethods:Accordingtotheexistenceofthalamicatrophy,the137patientswhohadcerebralencephalomalaciacausedbyvariantetiologieswereclassifiedintoatrophyicgroup(77cases)andcontrolgroup(60cases).Theauthorsanalysedtherelationshipofthemorphologyofcerebralencephalomalaciaandthalamicatrophy.Results:Theauthorsfoundthatthalamicatrophyshouldoc-curwhenthecerebralencephalomalaciainvolvedintheparietallobe,occipitallobeandbasalganglionarea.Thevolumeofencephalomalaciainatrophygroupwaslargerthanthatincontrol.Thedegreeofthalamicatrophywasnegativelycorrelatedwiththediseaseduration.Thalamicatrophyandthewaleriandegenerationofbrainstemusualycoexisted,buttheatrophycausedbyoccipitalencephalo-malaciawasseldomaccompaniedbythewaleriandegenerationofbrainstem.Discussion:Theau-thorsconsideredthatthalamicatrophycausedbyparietalencephyalomalaciacouldberelatedtoretro-gradedegenerationofnervecels,thatcausedbyoccipitalencephalomalaciarelatedtotheischemiaandnecrosisofthalamictissueandthatcausedbybasalganglionencephalomalaciarelatedtodirectdestructionofthalamusand/orretrogradedegenerationofnervecels.
