摘要
本文对160例缺血性心脑血管疾病患者的甲襞微循环(19项参数)和血液流变学(11项参数)进行了研究。结果显示:66.9%的患者甲襞微循环总积分异常,88.6%的患者血液流变学JB值异常。JB值与流态积分值和总积分值呈正性相关(P均<0.001,r分别为0.37和0.34),与襻周状态积分值相关性差(P<0.001,r=0.27),与形态积分值无相关性(P>0.05)。进一步分组研究显示:JB值正常组(31例)中,总积分值正常者占54.8%;JB值异常组(129例)中,总积分值异常者占72.1%;总积分值正常组(53例)中,JB值正常者占32.1%;总积分值异常组(107例)中,JB值异常者占86.9%。结果说明,大部分缺血性心脑血管疾病患者有微循环障碍和高粘滞血症,甲襞微循环与血液流变学改变之间存在正性相关,但并非完全相关。提示临床对缺血性疾病最好两项检查同时进行,并结合患者其它临床资料综合分析,提高临床诊断准确率。
A study was carried out on nailfold microcirculation(NFMC) and hemorrheology(HR) in 160 cases of Ischemic cardio-cerebral vascular diseases (ICCD). The results indicated that abnormal score values and JB values were found in 66.9% and 88. 6% patients, respectively. JB value was significantly correlated with the score values of hemodynamics (P<0. 001 , r = 0. 365), total score values(P<0. 001 ,r= 0. 342) ,and status of perivascular tissue(P<0. 01, r = 0. 27), negatively correlated with the score value of morphology(P >0. 05). Of 31 patients with normal JB values, 17(54.8%) had the normal total score values, 12(45. 2%) abnormal. Of 129 patients with abnormal JB values, 36(27. 9%) had the normal total score values, 91(72. 1%) abnormal. Of 53 patients with normal total score values, 17(32.1%) had normal JB values, 36(67. 9%) abnormal. Of 107 patients with abnormal total score values, 14(13. 4%) had normal JB values, 93(86. 9%) abnormal. It concluded that disturbance of microcirculation and hyperviscosity syndrome can be expected in the majority of the patients with ICCD. The disturbance of NFMC was positively correlated with hyper-viscosity syndrone, but no in the way of complete correlation. It was suggested that examination of both NFMC and HR should be simultaneously performed on the same patient and the results must be analyzed together with the clinical data and other results of test to improve the diagnostic rate.
