摘要
目的:观察高压氧(HBO)对茶碱药代动力学的影响。方法:6名健康成年人在正常状态和0.25MPa(2.5ATA)HBO状态下,分别单剂量空腹口服氨茶碱片300mg,给药后抽取静脉血,应用紫外分光光度法测定血药浓度。结果:HBO状态下的血药浓度较正常状态时偏低,所得药代动力学参数经配对t检验表明,两种状态下的消除半衰期和清除率具有显著性差异(P<0.05),但对其它药代动力学参数无显著性影响(P>0.05)。结论:在0.25MPa(2.5ATA)HBO状态下吸纯氧,可使体内氧浓度较正常状态高约12.5倍,从而牙致细胞色素P-450系统对茶碱的代谢加快,清除率增高。临床上使用氨茶碱合并HBO治疗时,应适时按个体化调整剂量,以控制其血药浓度,防止不良后果的产生。
Aim:To observe the effect of hyperbaric oxygen(HBO) on the pharmacokinetics of theophylline,Methods:Six healthy volunteers were given a single dose of 300 mg aminophylline orally under normal condition and 10d later under 1h 0.25MPa(2.5ATA)NBO(with 2 HBO/d ahead )re-spectively.Venous blood specimens were drawn right before and at 0.5,1,1.5,2,3,4,6 and 8 h af- ter the oral drug administration,and UV spectrophotography was used to determine the serum concen-trations of theophylline. Results:The serum concentrations of theolihylline under 0.25 MPa HBO con-dition were lower than that of normal and the elimination half life and clearance ratio of them showed significant difference(P<0.05)by the paired t-test.There were no significant changes of other phar-macokinetic parameters(P>0.05).Conclusion:Higher concentration of oxygen in human body under 0.25MPA HBO my cause the cytochrome P-450 system to metabolite and eliminate theophylline more rapidly. When patients receive repeated HBO treatments with aminophylline,it is appropriate to moni- tor the blood level so as to adjust its dosage individually in order to avoid untoward results.
