摘要
目的了解心脏介入手术中老年患者术前的情绪变化对手术的影响.方法采用HAMA、HAMD量表和临床自行设计问卷对105例老年患者手术前进行调查;记录病人术前、穿刺前、术中、术毕的血压和心率.结果 105例老年患者HAMA总分2～28分,平均(15.68±7.58)分,其中总分HAMA评分≥14分者74例,占70%;HAMD评分,总分3～27分,术前平均(16.87±8.56)分,HAMD≥17分者38例,占36%,其中冠心病组HAMA评分高于正常冠脉组P<0.01.自编问卷的6个项目存在情绪变化,心理问题的发生率均超过50%以上.心脏介入手术中病人在穿刺前、术中及术毕3个时点收缩压和心率明显高于术前.结论心脏介入手术中老年患者术前存在着不同程度的焦虑和抑郁情绪,影响手术的顺利进行及术后康复,手术前心理治疗有助患者的手术恢复和不良情绪的改善.
Objective To observe the effects of operation on the emotional changes of the aged patients before, heart intervention operation. Methods To carry out an investigation on 105 cases before heart intervention operation aged adopting HAMA, HAND measure forms and clinic self-edited questionnaire. To record heart rates and blood pressures before, during and after operation . Results Among 105 aged cases the HAMA marks of the patients were 2-28, the average was （15.68±7.58）, 74 cases of them had total≥14 marks, accounted for 70%. The HAMD marks of the patients were 3-27, the average was （16.87±8.56）, 38 cases of them had total≥17 marks ,accounted for 36 %. H AMD scores in the normal coronary arteries were lower than those in the coronary heart diseases. Heart rates and systolic pressures during and after operation were higher than before operation, in the self-edited questionnaire, the cases with emotional changes , the happening rate of psychological questions were exceed 50%. Conclusion The old patients before heart intervention operation had various degrees of worrying and depressed emotion. They had the effects on process of operation and the patients, recovery. Psychological treatment before operation is beneficial to the patients, recovery and the improvement of ill-emotion.
