摘要
目的:建立中国人血清N末端B型钠尿肽原(NT-proBNP)参考值。方法:采用RocheproBNP试剂盒,用电化学发光法在RocheElecsys2010全自动分析仪上测定442名健康中国成年人血清NT-proBNP浓度,建立参考值范围,并同时测定肝肾功能、血糖、血脂等指标,计算预测肾小球滤过率(eGFR),分析年龄、性别、eGFR等对NT-proBNP的影响。结果:NT-proBNP呈偏态分布。按每10岁一个年龄组及男女性别比较,60岁以下各年龄组之间差异无统计学意义,60岁以上2个年龄组的LgNT-proBNP均值均明显高于60岁以下各年龄组(P<0.05);各年龄组女性NT-proBNP水平均高于男性(P<0.05),全体女性NT-proBNP中位数水平(57.76pg/ml)也高于男性(23.42pg/ml)(P<0.001)。NT-proBNP与性别的相关性最密切(r=0.496,P<0.001),其次是年龄(r=0.330,P<0.001),与eGFR呈微弱相关(r=-0.328,P<0.001)。以60岁为分界年龄,以97.5%位数为参考值上限,中国人不同年龄、性别的NT-proBNP参考值为:≤60岁,男性<83.72pg/ml,女性<131.6pg/ml;>60岁,男性<158.17pg/ml,女性<205.54pg/ml。结论:年龄、性别、种族和地域可以影响健康人NT-proBNP分布,中国人需要建立自己的NT-proBNP参考值。
Objective To establish the reference values of serum NT-proBNP in healthy Chinese adults. Methods The concentrations of serum NT-proBNP in 442 healthy Chinese adults were determined by an automated electrochemiluminescence immunoassay on Roche Elecsys 2010. The impacts of age, gender and estimated glomerular titration rate (eGFR)on NT-proBNP were analyzed. Results There was no significant difference in the mean Lg NT- proBNP values among the groups with the ages of 20-30, 31-40, 41-50 and 51-60 years old. The mean value of Lg NT-proBNP in the groups with the age of over 60 years old was significantly higher than that in the groups with the age of under 60 years old (P〈0.05). Women had higher NT-proBNP concentration than men (median: 57.76 pg/ml vs. 23.42 pg/ml, P〈0.001). NT-proBNP showed positive correlation with sex (r=0.496,P〈0.001)and age (r=0.330,P〈0.001), and showed negative correlation with eGFR (r=-0.328,P〈0.001). The 97.5% percentile values were chosen to define the reference range in Chinese, that is, 〈83.72 pg/ml for men and 〈131.6 pg/ml for women under 60 years old, and 〈158.17 pg/ml for men and 〈205.54 pg/ml for women over 60 years old. Conclusions Age, gender, race and region may influence the reference values of NT-proBNP in healthy adults. It is necessary to establish the NT-proBNP reference values for Chinese people.
