摘要
目的研究不同剂量布比卡因用于老年患者腰麻的安全性和有效性。方法择期行下腹部及下肢手术老年病人40例,ASAⅠ、Ⅲ级,随机分成低剂量组和高剂量组,每组20例。穿刺点选择L3～4或L2～3椎间隙,以0.1mL/s的速率蛛网膜下隙注入局麻药2mL。记录麻醉各时点血压、心率变化,并观察用药后的不良反应。结果两组均能提供较为完善的麻醉。低剂量组血液动力学稳定,高剂量组有4例术中低血压伴心动过缓;低剂量组术中未发生恶心呕吐,高剂量组有2例术中发生恶心呕吐。术后尿潴留发生率,低剂量组极显著少于高剂量组(P<0.01)。结论布比卡因7.5mg腰麻用于老年病人是安全的,其循环和呼吸系统较稳定,并能达到较为完善的麻醉效果。
[Objective] To study the safety and effectiveness on the elderly patients spinal block with different dose bupivacaine. [Methods] 40 elderly patients who would be operated in the lower abdomen and lower extremity, ASA Ⅰ -Ⅲ grade. They were divided into low dose and high dose group randomly, each group were 20 patients. The puncture spot was choose in the spinal interapaee of Ls-4 or L2-3 ,2mL local anesthetic was injected into the subaraehnoid space at the velocity of 0.1 mL/s. The change of the blood pressure and heart rate were documented at the different time, and the side effect was observed after medication simultaneously. [Results] Two groups could provide better consummate anesthesia. The blood kinetics was stable in the low dose group, 4 eases had the hypotension and bradyearida in the high dose group during the operation; there was no nausea and vomiting phenomena in the low dose group, while two patients had nausea and vomiting in the high dose group. The retention of urine incidence was significantly less than the high dose group （P 〈0.01）. [Conclusion] 7.5 mg Bupivaeaine is safe in the elderly patients spinal block, the circulation and respiratory system are stable, which could arrive the better consummate block effective.
