摘要
目的探讨彩色多普勒血流显像技术对甲状腺功能减退心脏病形态学改变及血液动力学变化的诊断价值。方法使用彩色多普勒超声诊断仪对23例甲状腺功能减退心脏病患者与20例健康人心脏对比分析,并监测用甲状腺素治疗1a后,心脏形态学及血液动力学改变情况。结果发现23例甲状腺功能减退心脏病患者,出现心包积液为18例,占78.3%;心率减慢、心动周期延长为20例,占86.9%;CDFI显示室壁稍增厚>11mm为21例,占91.3%。甲状腺功能减退心脏病患者左心功能:以射血前期(PEP)延长为主,射血前期时间/射血时间(PEP/ET)比值明显增加;左室短轴缩短率(ΔD%)、射血分数(EF)和室壁增厚率(ΔT%)均降低。与20例健康人心脏对比差异有统计学意义(P<0.05或P<0.01)。监测用甲状腺素治疗1a后,100%的患者均好转,19例治愈,占82.6%,与20例健康人心脏对比差异无统计学意义(P>0.05)。结论CDFI对甲状腺功能减退心脏病形态学改变及血液动力学变化显示出一定的规律特点,为甲状腺功能减退心脏病诊断及治疗提供了依据,有着重要的临床应用价值。
Objective To study the use value of Color Doppler flow image（CDFI） echocardiography in diagnosing hypothyroid cardiopathy. Methods Using CDFI echocardiography, the variations of heart morpholongy and hemodynamics in 20 normal subjects and 23 patients with hypothyroid cardiopathy after thyroxin therapy were monitored. Results Pericardial effusion was shown in 18 cases （78.3 % ）, cardiac cycle in hypothyroidism longer than normal in 20 cases （86.9%）, ventricular wall thickening exceeding 11 mm in 21 cases （91.3%）.The pre-ejection period（PEP） was longer and pre - ejection period/ejection time（PEP/ET） ratio was higher in the patients than in normal controls （P〈0.01）. In hypothyroidi cardiomyopathy, the short axis shortening rate（△D% ）, the ejection fraction（EF）, and the ventricular wall thickeningrate（△T%） were all lower than those in normal controls （ P 〈 0.05）. The left ventrieular （LV） functions in all 23 cases were improved significantly after thyroxin theraphy for one year, of whom 19 cases werecured of LV dys functions（82.6% ）,and there was no significant difference between the 19 cases and the normal controls （ P 〉 0.05）. Conclusions CDFI eehoeardiography displays the structure of heart and dynamics of blood flow, mad it provides image evidences in diagnosis and treatment of hypothyroid cardiopathy.
