股骨颈骨折导针靶向定位装置的研制与临床初步应用 被引量:4

摘要 目的研制一种全新的、不依赖术中X线透视导航的股骨颈导针定位方法及装置.方法通过微创的手术方法,在股骨头颈的前方切一小口,打开前方关节囊,垂直插入一斯氏针于头颈交界的中点,作为一个真实的'靶',再于股骨大转子下作一纵行小切口,将已同'靶'连接的定位装置的导向套筒插入其骨皮质表面,引导导针通过股骨颈纵轴进入股骨头内.结果笔者在人体标本上进行了大量的模拟操作的基础上,临床验证38例(GardenⅡ~Ⅳ型),证明只要摆正患者体位,定位装置连接正确,击入导针则准确无误.术后手术台边摄X线片证实空心螺纹钉固定良好.结论本装置是一个全新的、微创的、价格低廉的医疗器械.适用于没有C型臂X线机的基层医疗机构或经验不多的骨科医师开展股骨颈骨折内固定手术. Objective To develop a completely new machinery location installationthat doesn' t need X - ray perspective navigation for the treatment of femoral neck fracture. Methods A small incision was first made by mini- trauma in front of the femoral head. A needle was inserted uprightly in themidpoint of the femoral head and neck border, and used as target. A longitudinal cut was then made in the greater trochanter of femur. The guidance sleeve that linked with target was inserted into the bone cortex surface, and leads the needle into the femoral head. Results After enough practice at human body specimen, the operations were performed clinically on 38 patients (2-4 according to Garden's classification). The postoperative radiography examination showed that the screw fixation was good. Conclusion This device is new and cheap, and caused minimal trauma. It's applicable in the grass- roots hospital in which no C- typed X-ray unit can be used for the internal fixation of femoral neck fractures.
