超声生物显微镜在穿透角膜移植25例中的应用 被引量:1

摘要 目的:探讨超声生物显微镜对穿透角膜移植术的指导作用。方法:回顾性分析2002/2004年于我院行穿透角膜移植的25例严重全角膜混浊患者。术前均行超声生物显微镜(UBM)检查。结果:25例患者中有4例角膜明显变薄(超过1/2角膜厚度),6例角膜水肿增厚、前房明显变浅,14例不同程度房角形态异常(2例房角完全关闭),5例虹膜局限性前粘连及2例瞳孔闭锁。角膜变薄,角膜水肿增厚、前房明显变浅,虹膜局限性前粘连及瞳孔闭锁均在术中得到证实。14例房角形态异常者术后8例发生暂时性高眼压,均予对症处理3~7d眼压恢复正常;2例房角完全关闭者术后出现持续性高眼压,需长期点用噻吗心安眼药水维持眼压20~25mmHg(1mmHg=0.133kPa)。结论:穿透角膜移植术前行超声生物显微镜检查对于指导术中操作、减少术中并发症以及判断预后等具有非常重要的价值。 AIM: To discuss the function of ultrasound biomicroscopy (UBM) in penetrating keratoplasty (PK). METHODS: Twenty-five patients with severe corneal opacity who have undergone PK during the year 2002-2004 in our department were retrospectively analyzed. UBM was performed on all the patients before operation. RESULTS: In the 25 cases, there were 4 cases with thin corneal, 6 cases with corneal edema, 14 cases with abnormal anterior angle (2 cases with completely closed angle), 5 cases with local anterior synechia irises and 2 cases with pupil seclusion. CONCLUSION: UBM is very important in directing surgical skills, reducing intraoperative complications and judging prognosis.
