摘要
目的:分析坚强内固定技术在自体骨移植修复下颌骨缺损中的应用,评估其效果。方法:收集1993年1月至2004年5月期间在武汉大学口腔医学院实施自体骨移植下頜骨重建手术中使用了坚强内固定技术并接受了随访的病人81例,观察病人术前术后的X线片,分析移植骨愈合情况。并对手术后病人下颌骨功能及其美观进行评估。结果:本组81例病人中共使用坚强内固定夹板367块。植骨手术成功79例(97.5%),病人下颌骨形态及其功能恢复良好。术后6个月摄片观察发现15例(18.6%)病人植骨部位存在"应力遮挡"现象,但未发现与之相关的严重并发症。结论:坚强内固定技术在自体骨移植下颌骨重建手术中对骨愈合及其移植骨的塑形有良好的作用,存在"应力遮挡"现象,但不会造成严重并发症,可以不必在骨愈合后常规取出钛板。
Objective： To investigate the technic of rigid fixation applied in mandibular reconstruction with autogenous bone and evaluate the efficiency of this technic. Methods： 81 patients, who received mandibular reconstruction with autogenous bone applying with rigid fixation in the School of Stomatology of Wuhan University from January 1993 to May 2004, were collected and underwent retrospective study. The clinical results and x - ray pictures were analyzed and the healing processes were evaluated. The functional and aesthetic results of the mandible reconstruction were also evaluated. Results ： In 81 patients who underwent mandibular reconstruction, 367 rigid fixation plates were inserted. There were 79 （97.5%） cases achieved successful effect. The mandibles form was recovered. The “stress shielding” was observed in 15 cases （18.6%） 6 months after operation without serious complications. Conclusions： The effects of rigid fixation applied in mandibular reconstruction were perfect in the healing and plasty of the transplant bone. “Stress shielding” can be observed without serious complications. Routine removal of the titanium plates after healing of the transplant bone was not advocated.
