摘要
目的 探讨亚低温联合高压氧治疗新生儿缺血缺氧性脑病(HIE)的临床效果.方法 将75例新生儿HIE患者随机分为高压氧联合亚低温治疗组(治疗组)40例和对照组35 例;对照组接受常规综合治疗,治疗组在此基础上行早期高压氧联合亚低温治疗.两组患儿在生后4、14 d进行新生儿神经行为测定,3、6、12个月进行婴幼儿智能运动发育检查测定.结果 两组患儿出生后4 d测定值比较差异无统计学意义(t=0126,P>0.05),生后14 d测定值高压氧组与对照组比较差异有统计学意义(t=2.48,P<0.05),3、6、12个月亚低温高压氧组智力发育指数和运动发育指数均高于对照组,两组比较差异有统计学意义(P<0.05).结论 早期应用亚低温高压氧治疗新生儿缺血缺氧性脑病有明显的近期和远期神经保护作用,可提高其生存质量,治疗效果肯定.
Objective To investigate the clinical effect of mild hypothermia complicated with heperbaric oxygen in the treatment of neonatal hypoxic-ischemic encephalopathy.Methods Seventy-five neonatal hypoxic-ischemic encephalopathy patients were randomly divided into mild hypothermia complicated with heperbaric oxygen group(40 cases) and the control group(35 cases).The newborns in control group were given routine and comprehensive treatment.The newborns in treatment group were given mild hypothermia complicated with heperbaric oxygen therapy on the basis of routine and comprehensive treatment.The newborns with HIE were estimated by neonatal behavioral neurological assessment (NBNA) after being born 4 days and 14 days and were examined by mental and psycho-motor development after being born 3 months,6months and 12 months.Results The NBNA score when the newborns were 4 days had no obvious difference between the two groups (t=0.126,P>0.05).The NBNA score when the newborns were 14 days had obvious difference between the two groups(t=2.48,P<0.05).The index of mental and psychomotor development when the newborns were 3 months,6months and 12 months in mild hypothermia complicated with heperbaric oxygen therapy group was obviously higher than that in control group.There were obvious difference between the two groups(P<0.05).Conclusions Mild hypothermia complicated with heperbaric oxygen therapy have obvious near ang long neurological protective action on neonatal hypoxic-ischemic encephalopathy and can improve the living quality.The treatment effect is positive.
