胰十二指肠切除术后早期肠内营养对胃壁起搏电位恢复的影响 被引量:1

Effects of early enteral nutrition after pancreaticoduodenectomy on recovery of gastric electricity

摘要 目的观察胰十二指肠切除术后早期肠内营养(enteral nutrition,EN)对胃壁起搏点电位变化的影响。方法选择期胰十二指肠切除术患者56例,随机分为治疗组肠内营养联合肠外营养(parenteral nutrition,PN)32例,对照组肠外营养(total parenteral nutrition,TPN)24例。治疗组于术后第2天起行肠内营养,每个患者均于术后第4天晨、第8天晨查胃电图(electrogastrogram,EGG)观察胃壁起搏电位中正常慢波的变化。结果治疗组及对照组患者术后胃电图均有不同程度紊乱;治疗组第4天晨胃电图餐前、餐后正常慢波百分比分别为33.2%、41.6%,对照组分别为10.4%、13.7%;治疗组第8天晨餐前、餐后正常慢波百分比分别为63.4%、77.6%,对照组分别为48.3%、66.2%。治疗组正常慢波百分比明显高于对照组(P<0.01或P<0.05)。结论术后早日行肠内营养对促进胃电恢复有重要意义。 Objective To study the effect of early enteral nutrition after pancreaticoduodenectomy on the recovery of gastric electricity. Methods Fifty-six patients after pancreaticoduodenectomy were randomly divided into two groups:the treatment group(n =32,enteral nutrition and parenteral nutrition)and the control group(n=24,parenteral nutrition).The treatment group started enteral nutrition the day after operation.The electrogastrogram was recorded for every patient before and after breakfast on the mornings of the 4th day and the 8th day after operation.Results Both the treatment group and the control group showed gastric disorder.In both groups,the percentage of normal'slow waves'were 33.2%,41.6%,10.4%,and 13.7% on the 4th day after operation,compared to 63.4%,77.6%,48.3%,and 66.2% on the 8th day.The percentage of normal'slow waves'in the treatment group were increased compared with the control group(P<0.01 or P<0.05).Conclusions Early enteral nutrition after pancreaticoduodenectomy is important in for recovery of gastric electricity.
