摘要
本文报告心肌梗塞后室间隔穿孔９例外科治疗。全组男性８例，女性１例，平均年龄５０岁。７例术前心功能为Ⅲ～Ⅳ级，其中１例合并心源性休克。全组８例择期手术，１例急症手术，手术死亡１例（１１％）。术后６例获随诊，心功能均转为Ⅰ～Ⅱ级，效果良好。结合文献复习，讨论了本病外科治疗的有关问题。
From 1972 to 1993,9 cases(8 males,1 female)with mean age of 50 yrs. underwent surgical repair for postinfarcion ventricular septal defect in Fu Wai Hospital. Preoperative cardiac function was NYHA class Ⅲ-Ⅳ in 7 cases including 1 case complicated by cardiogenic shock and class Ⅱin 2. The operation was performed on cardiopulmonary bypass.Ventricular septal defects were found single in 6 cases and multiple in 3. The repair techniques included septal patch (5cases), endocardiac patch (2 cases),and mattress suture(2 cases).Combined procedures included ventricular aneurysmectomy (7 cases) and coronary artery bypass grafting (6 cases). Postoperative IABP was needed in 5 cases for circulatory support. 8 cases survived and 1 died(emergency operation)with a mortality of 11%,6 cases were of cardiac function class Ⅰ-Ⅱin a mean follow up period of 20 months (3 months to 5 years).It is concluded that surgical repair for postinfarction ventricular septal defect is an effective treatment with good early and late results.
