摘要
利用血管镜直视的先进性能,在成功设计和施行了单瓣膜修复成形术的基础上,作者又进一步成功地施行了血管镜直视下股浅静脉多瓣膜壁外修复成形术。经20例24条下肢61对瓣膜手术的初步临床应用,证明该手术优于传统的和单瓣膜修复成形术。其优点具体体现为“壁外”和“多瓣膜”。“壁外”即指不切开静脉壁即可修复瓣膜,从而避免了瓣膜损伤和继发血栓形成。“多瓣膜”即指一次手术可同时修复两对以上瓣膜,以完善的多瓣膜的整合力量替代单瓣膜的薄弱屏障,极大地提高了对抗逆向静脉血柱压力的效能。血管镜直视的先进性能是实现上述优点的技术基础,它不仅使手术操作更为精确,而且可在直视下即时完成手术效果的评估。虽然作者详尽介绍了手术方法、注意事项和初步经验,但作为一个新设计术式,其科学性和实用性尚待远期疗效和更多作者的应用验证。
With the excellent property of gracile comliance and direct visionof angioscopy,and on the basisof design and performance of the sing-le external xal. uloplasty ( SEV ) , we further performed multiple exter-nalval, uloplasty (MEV) of the superficial femoral vein(SFV) .The ihi-tial experience of 61 valvuloplasties in 24 lower extremities of 20 patl-ents with primary venous insufficiency (PVI) verifies that MEV hasbatter effect than SEV and traditional vavuloplasty. The merits ofMEV are that valvuloplasty is realized without venotomy,which avoidsinjury of valves and diminished the rate of postoperative thrombosis,and that multiple valves are repaired at one time, which integratesthe multiple repaired valves to correct the venous reflux more effect-ively. The angioscopy can not only make the operation procedure moreprecise, but diagnose the PVI visually, also evaluate the operativeeffect immediately. with the introducton of the method, cautions andinitial experience, the author expects that this newly-designed MEVoperation procedure is tested by long-term results and other authoes'experlence.
