摘要
观察以改良颞肌束移位术矫治的麻风性兔眼12例,发现手术后闭眼时睑裂宽度<1mm者3人3眼,1～3mm者6人9眼,疗效优良者占66.7％;闭眼时睑裂虽为4～5mm,但较术前有明显改善者4人6眼,有效率100％。改良颞肌束移位术是对颞肌束移位加筋膜条悬吊术的改良,区别在于上股筋膜条通过上睑缘皮下隧道后缝接于上睑内中1/3交界处的睑板上,而并非缝于内眦韧带上,其优点是有利于调节张力,避免上下筋膜条因牵引点与着力点一致而造成睁眼困难。
Twelve cases of lagophthalmos with leprosy correced by using a modified temporal muscle bundle transposition have been evaluated, of which three eyes in three cases showed the width of the palpebral fissures in less than one mm at closing eye and nine eyes in six cases in one to three mm after the operation,making up 66. 7% ,and six eyes in four cases with the palpebral fissure of four to five mm had also some correction as compared with that before the operation. The difference between the modified and original methods is that the upper strand of the fascia strip is fixed on the tarsus at the junction of medial and middle thirds but not on the medial can-thus ligament through a tunnel in the upper palpebral margin,so as to modulate the tension of the fascia strip for averting possible difficulty in opening the eye because of coincidence of the points of traction and catching-force of two strands of the fascia strip.
