摘要
本文对严重开放性关节损伤治疗４ｌ例，认为早期高压冲洗创口，反复彻底清创是防止感染的重要环节。具体做法：对Ⅰ、Ⅱ度污染不严重的开放性关节骨折，彻底清创同时恢复解剖结构，坚强内固定，此有利于刨口修复和骨折愈合，并能实现早期关节功能锻炼。对大面积皮肤挫灭失活，彻底清创后植皮复盖创面。对粉碎性骨折，关节软骨严重损害的关节内有折，不能实施内固定者，彻底反复清创后，手术恢复其解剖结构，加用外固定，连续药物灌注冲洗。此乃预防和治疗关节损伤感染的好方法。
The article concluded retrospectively 4l cases suffered from seriours articular injury and put
forward to some new points :Washing with high pressure in early stage was a key step to
prevent infection.The patients with 、Ⅱdegree fracture without serious contamination were
given internal fixation and cases with large area of skin contusion were given skin grafting after
washing repeatedly.Internal fixation was not the main reason of infection.Severe comminuted
fracture with damage of articular cartilage could not be treated with internal fixation and should
be operated to restore its anatomic structure with external fixation.Washing with drug
continuously is a good method.Following up 4l cases show that satisfactory rate is
78%,functional limitation l4.6%,functional lose 4.9%and suppurative infection is 2%.
