慢性原发性肾小球疾病尿视黄醇结合蛋白的改变

摘要 尿RBP排泄量是近端肾小管损伤的灵敏指标。用双抗体夹心型酶免疫法测定本组72例原发性肾小球疾病患者尿RBP排泄量,慢性肾炎52例,其中22例血压增高;IgA肾病20例。结果表明,慢性肾炎患者尿RBP排泄量增加,血压增高的慢性肾炎患者尤甚。用开搏通治疗一月,血压控制在正常范围后尿RBP排泄量降低。提示慢性肾炎病人控制血压有十分重要的意义。IgA肾病患者尿RBP排泄量除4例增加外,与正常对照无差异,说明这些患者近瑞肾小管功能正常。 It has been suggested that urinary retinol-binding protein (RBP)excretion is a useful marker of proximal tubular disfunction, In this study, the concentraction of RBP in urine was determined by enzyme-linked immunosorbent assay in 72 patients with primary glomerular diseases, consisting of 52 case of chronic glomerulonephritis, in which 22 cases had hypertension, and 20 cases of IgA nephropathy diagnosed by renal biopsy. The results showed that urinary RBP excretion was increased in glomerulonephritis patients, especially in glomerulonephritis with hypertension. The concentration of urinary RBP in patients with hypertension was lowered after captopril treatment for a month, but was still higher than controls, suggesting that the control of blood pressure has an important implication in these patients.Urinary RBP excretion in IgA nephropathy was not different significantly from normal controls except for 4 patients with elevated urinary RBP, suggesting a normal proximal tubular function in patients with IgA nephropathy.
