摘要
目的探讨动脉灌注化疗治疗中晚期直肠癌的临床价值。方法将93例不能施行Ⅰ期根治术的中、晚期直肠癌患者,随机分为术前动脉灌注化疗组(研究组48例)和静脉化疗组(对照组45例),两组患者除术前化疗给药方式不同外,其它治疗相同,密切随访,观察患者术后生存情况。结果研究组症状明显减轻,肿块缩小,有效率(CR+PR)为83.3%。其中28例经化疗后能行Ⅱ期根治术,经随访5年生存率为45.8%,其中3例术后生存已超过8年。对照组有效率为40%(P<0.01),5例行Ⅱ期根治术,其余只能行姑息手术,5年生存率为22.2%(P<0.01)。结论动脉灌注化疗安全、有效,可改善直肠癌临床症状,抑制癌转移,提高手术切除率及生存率。
Objective To study clinical value on treatment of advanced rectal cancer with intra-arterial chemotherapy. Methods 93 cases of advanced rectal cancer patients with non-operation Ⅰ stage were randomly divided into pro-operative intra-arterial chemotherapy group （study group 48 cases ） and vein chemotherapy group （control group 45 cases ）. The two groups received the same medical treatments except with different way of chemotherapy. Both groups received intimate follow-up and were observed their survival situation. Results In the study group, the symptom was obviously alleviated and the tumor size was reduced. The effective rate （CR＋PR） was 83.3%.28 out of the 48 cases could implement operation Ⅱ stage after chemotherapy. The 5-year post-operative survival rate was 45.8%, and 3 patients were survived for more than 8 years. In the control group, the effective rate （CR＋PR） was 40% （P〈0.01）. 5 out of 45 cases could implement operation Ⅱ stage, while other patients implement palliative operation. The 5-year post-operative survival rate was 22.2% （P〈0.01）. Conclusion Intra-arterial chemotherapy is safe, effective and can ameliorate clinical symptoms of rectal cancer, restrain carcinoma metastasis, improve operative rate and survival rate.
