迷走神经对心房颤动影响的离子通道基础 被引量:12

摘要 迷走神经张力升高可以缩短心房有效不应期(AERP),升高AERP离散度,并诱发心房颤动(简称房颤),其末梢释放的乙酰胆碱(Acetylcholine,Ach)与心房的胆碱能M2受体结合,激活心房毒蕈碱激活钾通道(KAch),促进K+外流。由于KAch在左右房的分布密度不均一,导致极化恢复的离散升高,使心房内发生传导阻滞和不完全折返,这是诱发房颤的基础。Ach还可以影响ICa,L的钙离子流。一些药物直接作用于KAch,抑制IkAch,因此可以用于治疗与迷走神经有关的房颤。迷走神经和交感神经末梢释放的三磷酸腺苷对IkAch的影响可能是对心房肌电生理协同作用的基础。
