摘要
目的探讨前置胎盘的类型、临床表现与妊娠结局的关系。方法将39例前置胎盘分为轻度组(边缘性前置胎盘)8例及重度组(部分性前置胎盘及中央性前置胎盘)31例,对其临床表现及妊娠结局进行回顾性分析。结果重度组分娩时的孕周及新生儿体重明显小于轻度组(P<0.05);而两组初次出血的孕周、产前出血发生率、出血次数、大出血例数、产后出血发生率及剖宫产率差异无显著性(P>0.05);有产前出血组诊断及分娩时的孕周明显小于无出血组(P<0.01);两组剖宫产率差异无显著性(P>0.05)。结论前置胎盘类型与临床表现、阴道出血状况及妊娠结局有很大的差异,无典型的临床特征可循,不能简单地据此提出处理方案。
Aim To investigate relationship between the degree of placenta previa, clinical features and pregnant outcomes. Methods Records of 39 gravidas with placenta previa in our hospital during four years, mild previa group （marginal placental previa） 8 and severe previa group （partial placental previa and central placenta previa） 31 were reviewed retrospectively, and outcomes were compared. Results The gestational age at delivery and birthweight were significantly smaller in severe group than that in mild one（P 〈 0.05 ）. The gestational age at the first bleeding, the incidence of antepartum haemorrhage, the times of bleeding, the episodes of heavy bleeding, the incidence of postpartum haemorrhage and the number of deliveries by caesarean section in two groups showed no significant differences（P〉0.05 ） ; The gestational age at diagnosis and delivery were significantly smaller in women with antepartum bleeding than in those without （P〈0.01）,while the number of deliveries by caesarean section in two groups showed no significant differences（P〉0.05）. Conclusions The degree of placenta previa, clinical features, vaginal bleeding and pregnant outcomes are highly variable. There are no obvious clinical features that are typical in predicting the clinical outcomes. They can＇t be proof to determine the treatment plan simply.
