摘要
目的评价在下腔静脉滤器保护下手术取栓治疗下肢深静脉血栓的疗效和安全性。方法12例下肢深静脉血栓(deep vein throm bosis DVT)病人,先经对侧股静脉置入下腔静脉滤器于下腔静脉,然后手术取栓,术后溶栓、抗凝、去聚治疗。结果全部病人无肺栓塞发生,肢体肿胀消除满意,静脉造影5例深静脉主干通畅、7例以再通为主,滤器无移位。结论在下腔静脉滤器保护下静脉取栓然后溶栓是治疗下肢DVT安全、有效的方法。
[Objective] To estimate the effect of surgical thrombectomy after protective inserting Greenfield caval filter in acute deep vein thrombosis （DVT） of lower extremities. [Methods] 12 patients with acute DVT of lower extremities were teated by surgical thrombectomy after protective inserting Greenfield caval filter. Then we used the thrombolytic and anticoagulant therapy after operation. [Results] In all patients with DVT, there was no pulmonary embolism occurred and the tumescent eliminating was satisfaction. The postoperative angiography showed that successful recanalization in main venous vessel in 5 of 12 and recirculating mainly in main vein in 7 of 12 was achieved. [Conclusion] The surgical thrombectomy after protective inserting Greenfield caval filter and thrombolytic and anticoagulant therapy is a safe and effective method in the patients with acute DVT of lower extremities.
