摘要
本文应用免疫组化及ＰＣＲ－ＳＳＣＰ技术对５８例乳腺肿瘤ｃ－ｅｒｂＢ－２癌基因表达及ｐ５３抑癌基因突变进行研究，结果显示乳腺癌中ｃ－ｅｒｂＢ－２表达率为３８％，ｐ５３表达率为５２％，导管内乳头状瘤均为阴性；ｃ－ｅｒｂＢ－２表达与乳腺癌组织学分级（Ｐ＜０．０１）及淋巴结转移呈负相关（Ｐ＜０．０１），与雌激素受体状态呈正相关（Ｐ＜０．０１）；ｐ５３表达与淋巴结转移呈正相关（Ｐ＜０．０１），且倾相于≥２ｃｍ的肿瘤；两基因的表达均与乳腺上皮细胞的恶性转化及淋巴结转移密切相关，但两基因间无明显相关性；因此，每一基因的改变均可作为不依赖于淋巴结状态的独立预后指标。ＰＣＲ－ＳＳＣＰ分析表明，１５例ｐ５３基因表达阳性病例中４例于外显子５，６例于外显子７有该基因的突变（６７％），说明乳腺癌ｐ５３基因突变也多发生于其突变热点部位。
Exppressions of c-erbB-2 gene and p53 gene mutations were observed in 58 cases of breast tumors by using IHC, PCR-SSCP.The results showed 38% malignant cases were positive in immunohistochemistry staining for c-erbB-2 and 52% for p53,while intraductal papillomas were negative staining.c-erbB-2 expressions were positively associated with tumor grades(P<0.01)and lymph node metastasis(P<0.01),but negatively with ER status(P<0.01).p53 expressions were also positively asociated with lymph node metastasis(P<0.01)and tended to≥2cm breast cancers.The alterations of the two genes were significantly associated with cancer transformation of mammary epithelium and lymph node metastasis,but didn't show correlations with each other.Hence,alteration in each gene might be an independent prognostic index in evaluation of breast cancer.Analysis of PCR-SSCP of p53 gene showed that in 15 cases of p53 positive staining,change of polymorphisrn was identifiedin 4 of exon 5,6 of exon 7(67%).This informed that p53 gene change in breast cancer principaly involved in the hotspots of mutations.
