脂质过氧化在严重烫伤后肠源性感染发病机制中的作用 被引量:8

Role of Lipid Peroxidation in the Mechanism of Bacterial Translocation from the Gut after Severe Thermal Injury in Rats

摘要 本实验共用大鼠186只(体重180~220g)随机分为5组:正常对照,单纯肠道给菌,单纯烫伤,烫伤加肠道给菌,烫伤加肠道给菌并给予超氧化物歧化酶(SOD)治疗。管饲细菌为绿脓杆菌,均以异硫氰荧光素标记以便示踪,同时观察烫伤后回肠粘膜内脂质过氧化产物丙二醛(MDA)含量,回肠粘膜病理改变和肠道内细菌移位的动态变化。结果表明:给菌烫伤组和单烫组肠粘膜MDA含量显著高于单菌组和正常组,伤后12h达高峰;光镜和电镜观察发现,伤后24和48h病理改变较严重;给菌烫伤组肝脏和血流中标记绿脓杆菌检出数或检出率显著高于单菌组。应用超氧化物歧化酶(SOD)后,回肠粘膜MDA显著下降,病理改变明显减轻,肝脏内标记绿脓杆菌的检出数和检出率均降低,血培养未发现标记绿脓杆菌。本研究提示:严重烫伤后,回肠粘膜产生大量氧自由基,发生脂质过氧化作用与回肠粘膜屏障破坏有关,是引超肠道内细菌向深部器官播散的原因之一。应用SOD对保护肠粘膜屏障,减轻肠源性感染有一定作用。 Oxygen free radicals showing to play a role in ischemic-reperfusion-induced intestinal mucosal injuries, but the role of them in bacterial translocation from gut to extraintestinal sites is not documented. This experiment is performed on 186 rats (BW 180-220g) which are divided into 5 groups;normal control, simple Pseudomonas feeding, simple burns, combination of burns and Pseudomonas feeding with and without superoxide dismutase(SOD) treatment. The burnt animals receive 30% TBSA fullthickness scald burn, Pseudomonas fed ones are labelled with isothiocyanate for, tracing. Animals are sacrificed at 4, 12, 24, 48, or 72 h after injury. The changes in ileal mucosal malondialdehyde (MDA) are. measured, ileal mucosal pathological alterations are observed with light and electron microscopes. Bacterial translocation to liver and blood stream is traced. The combincation or simple burns result in a marked increase of ileal mucosal MDA(peak at 12h)followed by serious pathological changes;the incidence rate increases in the bacterial translocation from the gut; Animals treated with SOD was significant decrease in the ileal mucosal MDA, alleviation of the intestinal mucosal injury, reduction of bacterial translocation from the gut, and no labelled bacterium is isolated from the blood stream. Oxygen free radicals are demonstrated to be one of the causative factors in the intestinal mucosal damage after a severe thermal injury. SOD may preserve the intestinal mucosal barrier and inhibit bacterial translocation from the gut.
