摘要
报道５例先天性主肺动脉间隔缺损的外科治疗。本组５例中男２例，女３例。年龄１．５～８岁。中度肺高压２例，重度３例。３例伴有其它心血管畸形。Ⅰ型３例，Ⅱ、Ⅲ型各１例。笔者认为在体外循环下经主动脉切口，用Ｇｏｒｅ－Ｔｅｘ片修补缺损是有效的方法，避免了在主、肺动脉间作解剖分离和损伤冠状动脉口及主动脉瓣叶的危险，故优于结扎、切断缝合法。补片须略小于缺损，以防补片凸入肺动脉影响肺动脉血流。本病确诊后宜及早手术治疗，以防肺血管病变的发生。
This paper presents the surgical treatment of congenital aortopulmonary septal defect in 5 cases, typeⅠin 3 and typeⅡ and Ⅲ one each. Their ages varied from 1. 5 to 8 years.Moderate pulmonary hypertension(PH)was present in 2 cases and severe PH in 3.Three cases were associated with other congenital cardiovascular anomalies. The authors consider that repairing the defect with Gore-Tex patch through the ascending aortic approach under cardiopulmonary bypass is better and safer than ligation or division and suture because it avoids damage of coronary ostia and aortic leaflets.The authors also suggest once the diagnosis is established,the operation should be done earlier to prevent pulmonary vascular disease,and the patch must be a little smaller than the defect to minimize the hazard of obstruction of right ventricular outflow tract due to the convex of patch.
