摘要
为制作异常胰胆管连接(APBDU)最佳动物模型,笔者用23只犬分两组分别行胆总管→背胰管(13只)和背胰管→胆总管(10只)端侧吻合术。术前,术后5,10,15,30,50,80,120天和处死时(90~270天)动态检测血清淀粉酶、磷脂酶A2活性和甘胆酸含量,结合胰胆管测压、造影和病理切片等的观察表明:①本模型的胰胆管连接形态十分接近人类APBDU的解剖特点。②术后各犬无永久性吻合口狭窄且能保证持续性胆汁和胰液混流。③能诱发人类APBDU所合并的多种胰胆系病变。结论:本模型为符合人类APBDU的解剖和病理生理特点,能反映其病理演变规律的最佳模型。
In order to inquire into the best animal model of choledochopancreatic junction anomaly (CPJA), an end to side anastomosis of common bile duct to dorsal pancrea tic duct was performed in 13 dogs and the same anasomosis vise versa in 10 dogs. After operation, the phospholipase A2 activity, amylase activity and level of conjugated glycine bile acid in serum were measured on the 5th,10th, 15th, 30th, 50th, 80th and 120th day respectively, and cholangiopancreatography, manometry and pathological examination of biliary and pancreatic ducts were performed on 90th to 270th day. the results showed that: 1. this model was similar to human CPJA on anatomy; 2. there was no permanent narrowing at the anastomsis; 3. a free reflux of bile and pancreatic juice was permitted; 4. this model could induce many kinds of pancreatobiliary diseases associated with CPJA including cholecystitis, cholangitis, choledochol dilatation, pancreatitis and so on. This animal model is quite similar to the human CPJA in anatomy and physiology as well as in the pathologic pattern.
