摘要
瓣膜结构破坏越严重,经该瓣膜的反流血量越大,反流持续时间越长。依此原理,作者以双向彩超检测静脉的反流持续时间,经40例的检测证明以反流持续时间大于0.5s为诊断原发性下肢深静脉瓣膜功能不全(PVI)的阳性标准;采用站立位,气袖带压迫其胖肠肌(10.67kPa)诱发经静脉瓣膜反流,以脉冲式多普勒扫描和彩色血疏多普勒扫描检测反流的持续时间诊断PVI,敏感性为92%,特异性100%和精确性94%。
AbstractThe venous valve undergoes more dest ruction andmalfunction, and the quantity and duration of venousvalvular reflux are larger and longer. On the basis ofthis mechanism, we detected the popliteal vein refluxduration and various influencing factors with duplex ul-trasound.The reflux duration longer than 0.5 secandwas verified as the evidence of primary venous insuffi-ciencv(PVI)in the detection of 40 cases. The detect-ing method recommended by us is that with standingposition,and pneumatic compression 10.67kPa(80mmHg) applied to calf,the reflux duration throughpopliteal venous valves is detected by using bothpulsed-rave Doppler scanning and color-flow Dopplerscanning. With the rates of sensitivity(92%),speci-ficity (100%),and accuracy(94%),this noninvasive,quantitative,and easily handled method showed a verygood diagnostic value.
