摘要
１９９０年７月～１９９３年１０月应用Ｓｔｅｆｆｅｅ椎弓根螺钉钢板内固定系统治疗胸腰椎骨折脱位及腰椎滑脱５７例。术中采用在纵向对抗牵引脊柱的基础上垂直提拉椎弓根螺钉，徒手按压脊柱与反复交叉调整Ｓｔｅｆｆｅｅ钢板弧度的杠杆作用和螺旋机械功的动力拉顶复位相结合的方法，充分利用此内固定系统的“提拉螺旋”和“压榨螺旋”的机械功而获得理想的机械利益（Ａ＝２πＬ／ｈ）。本组采用３对椎弓根螺河固定３个椎骨的效果明显优于固定２个椎骨。固定的位置选择应有利于发挥杠杆作用，平均随访２１个月，术中能立即达到满意复位，平均改善程度为７０．３％。合并脊髓损伤的也有不同程度的恢复。
AbstractFrom July 1990 to October 1993,57 cases of tho-raco-lumbar fracture-dislocation and spondylolisthesiswere treated with Steffee,s operation。During the operation, under axial countertraction of the spinal co-lumn,the pedicle screw was lifted vertically,and thevertebra was pressed with the hand,The ideal mechanical benefit(A=2πL/h)was acquired by ade-quately using the“lifting spiral”and the“pressing spi-ral”mechanism。The selection of fixation of 3 verte-brae is remarkably better than that of 2 vertebrae,andthe site of fixation should be favorable for the full playof the“lever action”.The average follow-up periodwas 21 months,The improvement in average was70.3%。Those cases complicated with spinal cord in。jury also had improvements.
