摘要
报告用热或热药区域灌注治疗下肢恶性黑色素瘤５例，男４例，女１例；年龄３９～７３岁。肿瘤位于足４例，小腿１例，４例经髂外动静脉插管并切除腰交感神经节，１例经股动脉插管，灌注温度４１．５℃～４２℃。持续２小时，其中２例在灌注中加入ＤＤＰ（３．２ｍｇ／ｋｇ体重）。治疗后第１天即见肿瘤缩小，６天后所有病人的肿瘤均有明显缩小甚至消失，并可见不同程度脱色和萎缩，除１例术后小腿轻度肿胀，其它无不良反应。随诊２年除１例肺转移外，余未见病灶复发或转移。本组结果显示，热或热药区域灌注对治疗下肢恶性黑色素瘤有明显疗效，且保留了患肢，又提高了患者生存质量。
Five cases of melanoma in the lower extremity,4 male and 1 female aged 39-73,were treated with regional hyperthermia(3 cases)or plus DDP(2 cases)perfusion.The intubation was introduced by way of the external lilac artery and vein in addition to lumbar sympathectomy in 4 cases.In the other patient intubation was made in the femoral artery and vein.The perfusion was made at the temperature of 41.5～42.0℃ for 2 hours.All tumors reduced at the first day after perfusion. Six days later,all tumors apparently greatly reduced in size or even disappeared. A mild swelling in the involved leg appeared in one case and no other side effects were found.After 2 years follow up,4 patients were free of disease,one developed lung metastasis.This treatment was considered effective for melanoma with preservation of leg and maintenance of satisfying life quality.
