摘要
目的观察乌司他丁对肝叶切除术患者围术期细胞因子及肝功能的影响。方法28例肝癌行肝叶切除术患者,随机分为乌司他丁组(W组)和对照组(C组)。乌司他丁组按1万U·kg-1,于麻醉诱导后切皮前加入500ml林格液中静脉滴注。分别于诱导后切皮前(T1),阻断肝门10min(T2),恢复肝血流后20min(T3),恢复肝血流后1h(T4),术后1d(T5),术后3d(T6)抽取静脉血,行血浆谷草转氨酶(ALT)、谷丙转氨酶(AST)、总胆红素(TBIL)及肿瘤坏死因子-浕(TNF-浕),白细胞介素-6(IL-6)的测定。结果乌司他丁组的ALT、AST在T3、T4、T5均明显低于C组(P<0.05),TBIL在术后第1d显著低于C组(P<0.05),TNF-浕、IL-6在T3、T4、T5均明显低于C组(P<0.05)。结论乌司他丁能抑制肝叶切除术患者围术期细胞因子的释放,对肝功能有保护作用。
Objective To investigate the effects of ulinastatin on cytokine and liver function parameters in patients underwent hepatectomy. Methods Twenty-eight primary hepatic carcinoma scheduled for hepatectomy were divided into two groups, each group consisted 14 patients: the controlled group(group C)and ulinastatin group (group W). In group W, patient received ulinastatin 10000U.kg-1,Plasma levels of alanine transaminase(ALT), aspartate transaminase(AST), total bilirubin(TBIL), TNF-ɑ, and IL-6 were measured before operation(T_1), 10min after hepatic inflow occlusion(T_2), 20min after recovering hepatic inflow(T_3), 1h after recovering hepatic inflow(T_4), 24h (T_5), 3-day (T_6) after operation. Results In the T_2,T_3,T_4,T_5,T_6 time points, the plasma levels of ALT, AST, TBIL were lower in the W group than that in the C group. The plasma levels of ALT, AST in the T_3, T_4, T_5 time points in the W group was significantly lower than that in the C group(P<0.05). On the first postoperative day, the plasma levels of TBIL in the W group was significantly lower than that in the C group(P<0.05). TNF-ɑ﹑IL-6 in the T_3, T_4, T_5 time points in the W group was significantly lower than that in the C group(P<0.05). Conclusions The results suggest that ulinastatin can effectively holdback the release of cytokine and protect liver function in patients underwent hepatectomy.
