摘要
目的探讨传统内固定手术治疗股骨颈骨折愈合率低的主要原因,如何有效的消除骨折后产生的剪切应力,将剪切应力转变为压应力以及增加股骨头的血供是保证骨折愈合的关键因素。方法作者设计了股骨颈“V”形截骨、头颈嵌插、加压螺钉固定、股方肌骨瓣移植术治疗移位型股骨颈骨折21例。结果经3年以上随访,其中1倒股骨头轻度坏死塌陷,所有病例骨折全部愈合,功能恢复满意。结论该手术方法治疗股骨颈骨折可促进骨折愈合,降低了股骨头缺血坏死的发生率。
Objective To study the main causes of low cure rate of femoral neck fractures treated by traditional internal fixation,how to effectively eliminate the shearing stress after fracture.Ensuring fracture healing,turning shearing stress to compression stress and increasing blood supply to femur are the pivotal factors.Metheds A new operative method of 'V' shaped osteotomy over the femoral neck with impaction of femoral head and neck,tightly screwed intemal fixation and implantation of quadratus femoris′s bone petal,is designed and performed in 21 cases with femoral neck fracture.Results A follow-up period lasting more than 3 years showed slight necrosis in one case and fracture healing with satisfactory function recovery in all cases.Conclusion This operative method treating femoral neck fracture can promote fracture healing and reduce the incidence of ischemia and necrosis of femoral head.
