摘要
目的:观察兔下颌骨即刻牵引术后新骨形成情况,并与延迟期骨牵引相比较。方法:实验于2004-05/09在第四军医大学口腔医院颌面外科实验室完成。将18只兔随机分成3组,每组6只。即刻牵张组与延期牵张组均行双侧下颌骨皮质骨切开术,即刻牵张组术后即刻开始骨牵引,延期牵张组术后7d开始骨牵引。两组均牵引10d,2次/d,0.5mm/次,牵引期结束后继续以牵引器固定8周。固定期结束后处死动物,标本行大体、组织学、放射学观察,并进行三点抗弯实验。对照组不进行干预。结果:实验兔3组18只均进入结果分析。①大体测量即刻牵张组和延期牵张组下颌骨平均延长9.5mm,即刻骨牵拉术后下颌骨成骨明显。②X射线示骨间隙内新生骨已基本连接骨缺损,可见骨小梁结构,骨化明显,无骨不连和畸形愈合。③组织学见大量新骨形成,部分形成板层骨,新骨形成以膜内成骨为主。④三点弯法测试牵引区抗弯强度,即刻牵张组成骨区的抗弯强度与延迟牵引组和对照组3组的两两比较数据无差异犤(65±18.3),(193±15.4),(197±19.5)MPa,P>0.05犦。结论:兔下颌骨即刻牵引术后新生骨质的形成与延迟牵引术后的骨质形成无明显差异。延迟期的长短不会改变下颌骨牵引成骨中新骨形成的特性,在颅颌面骨的牵引中,延迟期并无明显必要。
AIM:To observe the formation of new bone in rabbit mandible following immediate distraction in comparison with that following delayed distraction. METHODS:The experiment was conducted in the surgical laboratory,Department of Oral and Maxillofacial Surgery,Stomatological College,Fourth Military Medical University from May to September 2004.Eighteen rabbits were randomly and equally divided into 3 groups.Immediate distraction group and delaying distraction group were performed corticectomy in bilateral mandibles.Distraction performed immediately after surgery in the immediate distraction group,and performed 7 days after surgery in the delaying distraction group.In the two distraction groups,the extension apparatus was stretched by 0.5 mm once,two times per day for 10 days totally,followed by another 8 weeks of fixation with the extension apparatus after distraction.After fixation period,all the animals were sacrificed.Then gross,histological and X-ray observations were performed to examine the specimens of mandibles,and meanwhile,3-point bending test was used to assess mechanical property.The animals in the control group was not intervened. RESULTS:All the 18 animals were involved in the result analysis.①By gross observation,the mandibles were extended by 9.5 mm on average in the immediate distraction group and delaying distraction group,and the formation of new bone was more obvious in the former group.②In the X-ray examination,defective bone was connected by new bone and bone trabecula could be observed;Ossification was obvious without bone nonunion and malunion.③In the histological observation,new bone was obvious and lamellar bone could be seen partially.The new bone was mainly formed by intramembranous ossification.④After 3-point bending test,the counter-bending strength was insignificantly different between any two of the three groups[(65±18.3) MPa in immediate distraction group,(193±15.4)MPa in the delaying distraction group and(197±19.5) MPa in the control group,P >0.05]. CONCLUSION:The formation of new bone in rabbit mandibles was unremarkably different between the immediate distraction and delaying distraction.Long or short duration of delaying distraction can not influence the mechanical property of new bone in the mandibles following distraction osteogenesis,so the delaying period is inconspicuously necessary during distraction of mandibular bone.
