摘要
目的:疼痛相关受体及电压门控离子通道中一些新的亚型在疼痛的发生发展中将被揭示出靶点的作用。资料来源:应用计算机检索Pubmed数据库2000-01/2004-11相关疼痛治疗靶分子的文章,检索词“pain,Nav1.8,N-typecalciumchannel,NR2Bsubunit,P2X3receptors,Glycinereceptors”。资料选择:对资料进行初审,纳入标准:①研究靶分子在疼痛中的作用及应用的文献。②不排除是否为随机、盲法等论证推荐的文章。排除标准:综述类文献及重复研究。资料提炼:收集到的资料中分别对疼痛机制中电压门控离子通道和受体进行了探讨。电压门控离子通道包括河豚毒不敏感Na+离子通道、N型Ca2+通道;受体有N-甲基-D-天冬氨酸受体、P2X3嘌呤受体及甘氨酸受体等。资料综合:针对疼痛机制的干预研究有的已进入临床试验(如N型Ca2+通道阻断剂),有的正在进行动物试验(如NR2B受体的拮抗剂)。也有一些研究应用基因治疗的手段干扰这些分子的表达,从而达到镇痛目的(如P2X3,Nav1.8)。结论:疼痛是一个多分子参与,多个水平调节的生理病理过程。除受体及电压门控离子通道学说外,还有辣椒素受体、乙酰胆碱受体、大麻素受体等多种分子参与疼痛发生过程,针对这些疼痛相关分子的研究有利于进一步了解不同分子之间的联系,探讨疼痛发生的机制和研制特异性干预措施。
OBJECTIVE:To reveal a variety of different isoforms of voltage gated ion channels and painrelated receptors served as the drug targets in the occurrence and development of pain.DATA SOURCES:The articles about drug targets of pain treatment published between January 2000 and November 2004 were searched for in Pubmed database,using the Keywords“pain,Nav1.8,Ntype calcium channel,NR2B subunit,P2X3 receptors,Glycine receptors' with computer.STUDY SELECTION:The articles were primarily checked up according to the inclusion criteria:①The articles concerning the role of the target molecules in pain and their application;②No matter the recommended articles based on randomized or blind proofs, and also according to the exclusion criteria:review articles and repeated studies.DATA EXTRACTION:Voltage gated ion channels(such as tetrodotoxinresistance sodium channel and Ntype calcium channel) and receptors[P2X3 purinoceptor,NmethylLaspartate(NMDA) receptor and glycine receptor]in the mechanism of pain were evaluated in the collected articles.DATA SYNTHESIS:Some of the intervention studies on pain mechanism have been conducted in clinical trials(antagonist of Ntype calcium channels).Some were progressing in animal experiments(antagonist of NR2B receptor).At the same time by using of gene therapy,investigators had interfered the expression of such molecules as P2X3 and Nav1.8 and gained successful analgesia.CONCLUSION:Pain is a physiopathological process involving multiple molecules and multiplelevel regulation.Besides the theory of receptors and voltagegated ion channels,the target molecules including capsaicin receptor,acetylcholine receptor,cannabinoid receptor and so on also participate in the occurrence of pain,and the studies on them will benefit the interrelation of different molecules,evaluate the pathogenesis of pain and investigate specific interventional measures.
