摘要
目的:采用简易智力状况检查法与西方失语症成套测验对脑损伤后失语症患者进行检查,探讨脑损伤后认知功能障碍与失语症患者语言功能的关系。方法:选择1997-03/2004-09在解放军总医院康复医学科接受住院或门诊治疗的脑损伤后失语症患者52例,男44例,女8例。均接受简易智力状况检查及西方失语症成套测验中失语商的检查,其中35例患者只接受西方失语症成套测验中操作商和大脑皮质商的检查。根据检查结果,计算出失语商、操作商和大脑皮质商。对简易智力状况检查法总分与西方失语症成套测验中的各亚项分值及失语商、操作商、大脑皮质商之间的关系分析采用Pearson相关分析和逐步线性回归分析。结果:按意向处理分析,52例失语症患者全部进入结果分析。①简易智力状况检查法总分与西方失语症成套测验中自发言语、听理解、复述、命名、阅读、书写、运用、结构各项分值有显著性相关关系[(11.88±7.06,11.38±5.12,117.83±50.03,70.60±32.96,39.13±25.97,58.46±19.65,41.69±22.36,50.51±7.56,69.34±18.41)分,(r=0.490~0.859,P<0.01)],经逐步判别分析入选回归方程的测验指标是命名、书写和复述。②简易智力状况检查法总分与西方失语症成套测验的失语商、操作商、大脑皮质商有显著性相关关系[(11.88±7.06,56.43?
AIM:To detect the patients with aphasia following brain injury by using mini mental state examination and western aphasia battery,and study the association b etween the cognitive functional impairment and the language functional impairmen t after brain injury. METHODS:A total of 52 patients(male:44 cases,female:8 cases) with aphasia foll owing brain injury were selected from Department of Rehabilitation Medicine,Gene ral Hospital of Chinese PLA from March 1997 to September 2004.They received mini mental state examination and western aphasia battery,among them,35 cases only received examination of operative intelligence quotient and cerebral cortex inte lligence quotient in western aphasia battery.According to the result,we calculat ed the aphasia intelligence quotient,operative intelligence quotient and cerebra l cortex intelligence quotient.The association between scores on mini mental st ate examination and western aphasia battery,aphasia intelligence quotient,opera tive intelligence quotient and cerebral cortex intelligence quotient was analyze d by using Pearson analysis,stepwise linear regression analysis. RESULTS:According to intention to treat analysis,all 52 patients were involv ed in the result analysis.①There was obvious correlation between total scores o f mini mental state examination and scores of spontaneous language,auditive und erstand,repeat,denomination,reading,writing,management and structure in western aphasia battery (1.88±7.06,11.38±5.12,117.83±50.03,70.60±32.96,39.13±25.97, 58.46±19.65,41.69±22.36,50.51±7.56,69.34±18.41,r=0.490-0.859,P< 0.01).Among the significant variables,denomination,writing and repeat entered into regressi on equation.②There was obvious correlation between total scores of mini mental state examination and scores of aphasia intelligence quotient,operative intelli gence quotient and cerebral cortex intelligence quotient in western aphasia batt ery (11.88±7.06,56.43±24.11,25.37±5.86,64.05±16.10,r=0.744-0.849,P< 0.01).A mong the significant variables,cerebral cortex intelligence quotient entered int o regression equation. CONCLUSION:Mini mental state examination is mainly used in preliminary screen ing for verbal intelligence quotient.But the total scores can reflect cognitive functional impairment in patients with aphasia,which indicates cognitive functio nal impairment exists in patients with aphasia.We should pay attention to cure o f cognitive function,besides routine speech therapy in aphasia.The correlations of aphasia intelligence quotient,operative intelligence quotient and cerebral co rtex intelligence quotient indicate effective correlation exists in language fun ction and cognitive function.
