大样本老年病人麻醉的病例分析 被引量:8

摘要 目的探讨老年病人的麻醉特点.方法2 039名老年病人,男1 351例,女688例,年龄最大者93岁,ASA分级主要是Ⅰ~Ⅱ级(1 740例),占总病例数的85%,约1/3的病人并存有内科疾患.主要为普外科和泌尿外科手术(75%),主要麻醉方式为全麻、硬膜外麻醉(88%).采集整个手术过程中的循环参数、总输液量、手术时间、硬膜外麻醉首次用药剂量,全麻的麻醉诱导、维持麻醉,计算每小时的输液量等.结果术中生命体征、总输液量随着年龄的增长逐渐减少,而单位时间内的输液量没有显著性差异,硬膜外麻醉首次用药量随着年龄的增长用药量逐渐减少.全麻病人的诱导量和维持量也逐渐减少.结论硬膜外麻醉初量为(14.90±3.18)ml,中位比低位硬膜外麻醉的用量少;全麻的诱导和维持量比年轻人少;其他的麻醉方式也比年轻人用量少;每小时输液量维持约1 000 ml,可基本维持循环功能的稳定.
