摘要
目的分析脑膜瘤的上皮型、纤维型、血管瘤型、间变型等4个常见亚型肿瘤实质和瘤周水肿区的血供,评价动态增强磁化率MR(dynamicsusceptibility-contrastMR,D)SC灌注成像技术表达不同亚型脑膜瘤灌注特点的可行性。方法对33例经病理证实的脑膜瘤患者行手术前DSCM灌R注成像扫描。测量肿瘤实质部分及瘤周水肿区最大相对脑血容积(relativecerebralbloodvolurmCeB,V)值及相应部位相对平均通过时间(relativemeantransittime,rM数TT值)并将结果进行分组统计分析。结果血管瘤型脑膜瘤实质部分的最大rCBV值高于其它亚型,有统计意义差异(方差分析,P<0.05);间变性脑膜瘤瘤周水肿区的最大rCBV值高于其它亚型,有统计意义差异(方差分析,P<0.05);各亚型间rMTT值无统计意义差异。结论DSCMR灌注成像对脑膜瘤的分型有临床价值。
Objective To evaluate the role of DSC MR perfusion imaging in pre-operation subtyping of meningiomas on the basis of differences in their vascularity inboth tumor parenchyma and peritumoral region. Subtypes of meningiomasinclude endothelium, fibroid and angioblastic. Methods 33 patients with meningiomasunderwent DSC MR perfusion imaging before resection. Maximum relative cerebralblood volume (rCBV) and corresponding relative mean transit time (rMTT) values ofthe same part in both parenchyma of meningiomas and peritumoral region were calculated.Data were analyzed group by group. Results Maximum rCBV in parenchyma ofangioblastic subtype was significantly higher than that of other subtypes (ANOVEanalysis, P < 0.05); maximum rCBV of anaplastic subtype in the peritumoral regionswas significantly higher than of other subtypes (ANOVE analysis, P < 0.05). rMTTvalues of all subtypes were not significantly different. Conclusion DSC MR perfusionimaging is clinically useful in subtyping of meningiomas.[
