准分子激光光学角膜切削术治疗近视115例7年疗效分析

摘要 目的评价准分子激光光学角膜切削术(PRK)治疗不同程度近视的效果。方法应用日本生产的NIDEKEC-5000型准分子激光机,对不同程度的近视眼(共115例208只眼)进行治疗。术前按屈光度(等值球镜)高低分三组:A组-0.5~-6.00D,162只眼;B组-6.12~-8.00D,28只眼;C组>-8.00D,18只眼。术后7年随访结果。结果三组裸眼视力≥1.0分别为99.38%、85.71%、16.67%;裸眼视力≥0.5分别为99.38%、96.43%、38.89%;屈光度在±1.00D分别为96.30%、82.14%、27.78%。A、B组疗效明显高于C组(P均<0.01)。结论PRK对治疗-8.00D以下的近视是安全有效的,预测性高,稳定性好,但对-8.00D以上的高度和超高度近视因存在屈光回退,部分患者的预测性和稳定性较差。 Objective To evaluate the clini cal efficacy of excimer la ser photorefractive keratectomy (PRK) in treatment of myopia of different degree . Methods An excimer laser machine (NIDEK, type EC-5000, made in Japan) was used to do PRK for treatment of myopia (115 cases, 208 eyes). Before operati on, all the patients were divided into three groups according to the diopter (sp herical equivalent): group A (-0.5~-6.00D, 162 eyes), group B (-6.12~-8.00D, 2 8 eyes) and group C (>-8.00D, 18 eyes). 7-year follow-up had been done. Results In groups A, B and C, the rates of uncorrected v isual acuity ≥1.0 were 99.38%, 85.71%, and 16.67%, respectively; ≥0.5 were 99. 38%, 96.43% and 38.89%, res pectively; the diopter within ±1.00D were 96.30%, 82.14% and 27.78%, respective ly. The curative rates in groups A and B were higher than that in group C (all P <0.01). Conclusion PRK is a safe, effective, pr edictable and stable procedure to correct lower and moderate myopia (<-8.00D). But predictability an d stability is not so good in high and extra-high myopia (>-8.00D) for high rate of refractive regression and respond of Haze.
