摘要
采用自体纤维蛋白粘合剂固定内植法鼓膜成形术中的移植物以减少术后新鼓膜前方裂孔的发生４０耳，结果优于对照组。对其机理进行了讨论。
AbstractWe have carried out a prospective studyon autologous fibrin adhesive ( AFA ) in my- ringoplasty with underlay technique in orderto clarify whether adding AFA betweenfascia and the remnant of the lympanicmembrane results in fewer primary recurrentperforations compared with the routinemethod.80 ears(73 patients)with dry anteriorperforations and without cholesteatomatouschange of the middle ear were operated onby one surgeon. Half of the ears were trea- ted wih AFA and the other without. Thechoice of the method was completely rando- mized.At follow up 3 months after surgery,otomicroscopy were performed. There wree 8ante rior reperforations in the control groupand only 1 in AFA group,The difference wassignificant(P<0.05).Gluing the graft to theremnant of the tympanic membrane (or to theintersurface of the annulus in total pertora-tions) , AFA mayprevent the graft from shif-ting caused by the movement or shrinkage ofthe packing beneath the graft so that theblood supply of the graft can be ensured.Webelicve that AFA may increase the take-rate of myringoplasty with underlay techui- gue.
